Job Description

Job Purpose

Reports to the ED on any clinical service concerns and provide clinical consultation and expertise to the AL and Horizon’s Wellness Directors. Collaborates with the Team Member Service Director to coordinate identified training requirements in accordance with state and federal regulations.

Duties & Responsibilities

     Resident Services

  • Completes potential new move-in and returning resident assessments prior to admission
  • Completes new move-in and returning resident paperwork
  • Completes PCS online service plans and care plan. Adheres to each residents care plan, updating the care plan as changes in each residents condition occurs and in accordance with state regulations
  • Coordinates with Wellness Directors on implementation and follow up to pharmacy consults
  • Performs monthly med cart audits in conjunction with AL/HZN Wellness Directors
  • Completes one med pass observations on each shift per month
  • Audits 25% of resident charts on a quarterly basis
  • Completes weekly EMAR audits (holes, exceptions, FSBS readings, etc.)
  • Reviews and submits weekly reports to ED, DRCS, and DQAC
  • Establishes and maintains a good relationship and open communication with residents, family members, team members, physicians, and vendors
  • Coordinates dining experience needs with Dining Services Director
  • In coordination with the Wellness Directors, ensure that all systems for providing and tracking resident care are completed in accordance with the Ridge Care policies and procedures
  • Oversees implementation of Shared Risk Reduction Programs
  • Oversees New Order Notebook, Coumadin Management notebook, and Diabetic Management
  • Oversees Resident Nurse Tracking Tool
  • Oversees monthly weight tracking tool
  • Coordinates with Wellness Directors on implementation and follow up to quarterly LHPS reviews
  • Participatse in Care Plan meetings for all residents
  • Completes wellness assessment on all residents every 6 months
  • Conducts and oversees:
    • 10/15 Medication Training
    • Diabetic & Coumadin Education
    • LHPS (staff, new move in, significant change)
    • PCS Care Plans
    • PCS Online Service Plan
    • Infection Control Training
    • Complete Monthly Audits of all resident receiving Coumadin and FSBS/Insulin
    • Review all residents identified as Fall Risks based on weekly At-Risk reports and work in conjunction with AL/HZN Wellness Directors to provide training on identified trends and patterns
    • Med Tech initial check-off
    • PCS Independent assessments and mediation
    • Responsible for administering and reading all TB’s for staff and residents

     General Management

  • Ensures paperwork that is required to be completed by a RN is up to date and completed in required time frame
  • Assumes on call responsibilities on a 24/7 basis in regards to clinical situation including but not limited to; residents sent to hospital, falls with fractures, resident deaths, medication errors, medications unavailable for administration, resident elopement, resident illness impacting three or more residents.
  • Assists with marketing of the community including developing relationships with local referral sources

    Consultation Responsibilities

  • Leads by example to encourage teamwork, promote Ridge Care philosophy and provide ongoing coaching to employees
  • Provides training of all team members in accordance to the company’s policies , training programs, and state and federal regulations
