    Ridge Care Senior Living is seeking a dynamic RN for our Resident Services Consultant position who will provide support, training, and direction at Deerfield Ridge Assisted Living located in Boone, NC and Forest Ridge Assisted Living located in West Jefferson, NC. This Full-time position offers a competitive salary and an attractive benefits package.

    Job Description:

      • Resident Care Consultant will be available for consultation if needed during the prescreen process.
      • Review required paperwork for all new move ins and readmissions within 7 days.
      • Complete annual PCS online service plans and PCS beneficiary Care Plans.
      • Provide oversight and review of all pharmacy consult findings.
      • Perform monthly med cart audits in conjunction with Assisted Living/Memory Care Wellness Directors.
      • Complete a wellness check on all residents in the community every 6 months.
      • Complete one medication pass observation on each shift per month.
      • Complete a monthly and as needed blood sugar audit of all diabetics in the community.
      • Complete a monthly Coumadin audit of all residents on Coumadin.
      • Participate in Care Plan meetings as needed.

    Conducts and oversees:

    – 10/15 Medication Training

    – Diabetic & Coumadin Education

    – LHPS (staff, new move in, significant change)

    – PCS Care Plans

    – PCS Online Service Plan

    – Infection Control Training

    • Complete Monthly Audits of all resident receiving Coumadin and FSBS/Insulin.
    • Review Weekly repots and submit to DRCS.
    • Review all residents identified as Fall Risks based on weekly At-Risk reports and work in conjunction with Assisted Living/Memory Care Wellness Directors to provide training on identified trends and patterns.
    • Med Tech check-off (Initial and as needed for follow-up training).
    • Participate in PCS Independent assessor assessments and mediation when on site.
    • Responsible for administering and reading all TB’s for staff and residents.
    • Ensure paperwork that is required to be completed by a RN is up to date and completed in required time frame.
    • Assist with marketing of the community including developing relationships with local referral sources.
    • Assume on call responsibilities on a 24/7 basis in regards to clinical situation including but not limited to; residents sent to hospital, falls with fractures, resident deaths, medication errors, medications unavailable for administration, resident elopement, resident illness impacting three or more residents.

    Qualifications Mandatory Requirements
